I hereby certify that my dog is in good health and has not been ill with any communicable condition in the last 30 days. I agree to not bring my dog to Katahdin Kritters Pet Resort (KKPR) if they show signs of illness (vomiting, diarrhea, coughing, limping, etc.) I further certify that my dog has not harmed or shown aggressive or threatening behavior toward any person or any other dog, and that my dog is being admitted based on this representation.

I understand that I am solely responsible for any harm to other people present while at this facility, as well as to other dogs caused by my dog while attending KKPR.

I further understand and agree that KKPR and their staff will not be liable for any problems that develop, provided that reasonable care and precautions are followed, and I hereby release them from any liability of any kind whatsoever arising from my dog’s attendance at the facility.

I am fully aware that vaccines, including Bordatella (kennel cough) do not protect against ALL communicable diseases. We highly recommend all dogs have a current Bordatella vaccine, but we cannot guarantee your dog will not contract this virus should there be an outbreak at our facility.

I understand that even though pets are very closely monitored, that does not guarantee your dog may not come home with a few play marks. There are always risks involved, including scratches and cuts, which are commonplace due to the nature of dog play. We DO NOT allow rough play, but we do allow them to play together. We will inform you of any injury that we are aware of; however, we may not see all scrapes and scratches that may occur here at KKPR. I hereby give permission for employees of KKPR to administer routine first aid treatment if necessary.

There is inherent rough-housing in play. All dogs are screened for aggression, but fights can and will still occur! EVERY dog has a bite threshold and has the potential to bite. It is also possible for a puncture wound to happen during play sessions. In the event of a major injury, we will contact you or your emergency contact listed on the registration form, so your veterinarian can be consulted. If possible, and with your permission, we will transport your dog directly to your vet for treatment. However, you will be responsible for all bills incurred because of the injury.
